Three things guide these efforts: engaging in open, honest dialogue, acting in transparency, and delivering on our commitment to advancing science and improving health.</p> <p>To help guide this work, the Novel and Exceptional Technology and Research Advisory Committee (NExTRAC) recently undertook a multi-year effort to provide the 51视频 with a roadmap for incorporating public voices in clinical research (<a href="https://osp.od.nih.gov/wp-content/uploads/2025/12/NExTRACs-ENGAGE-Report_FINAL.pdf">see NExTRAC report</a>).  Deliberations were informed by a team of <a href="https://partnersinresearch.nih.gov/members/">multi-disciplinary experts</a> and <a href="https://partnersinresearch.nih.gov/community-conversations/">community conversations across the country</a>, specifically asked to think about strategies for incorporating public voices at every stage of the clinical research process.</p> <p>I am pleased to endorse these recommendations, which focus on:</p> <ul> <li>providing a clear vision and framework for maximally involving patients and communities in clinical research;</li> <li>ensuring that people and communities have meaningful input into the agenda and direction of research that is relevant and impactful for them; and</li> <li>increasing transparency for how research participant data are utilized in moving the scientific enterprise forward.</li> </ul> <p>51视频 is eager to begin implementing this roadmap, starting in 2026. First, 51视频 will work with the community to make the return of participant research results standard practice. Initial efforts will focus on returning summary-level research results, with the intent of augmenting practices that result in returning individual-level research results to research participants. Providing individuals with their research data so they can make their own healthcare decisions is one of my top priorities as 51视频 Director.</p> <p>51视频 is also working towards harnessing the widespread adoption of electronic health records and other large scale electronic clinical information systems to accelerate biomedical research in ways not previously possible. In the spirit of transparency, <a href="https://osp.od.nih.gov/policies/healthcare-research/">51视频 is establishing agency-wide principles</a> to foster, promote, and guide the responsible conduct of research using clinical data. These principles serve as an initial transparent foundation for building trust with the public in how their data are respected and shared to drive improvements in health across the country.</p> <p>I want to thank the NExTRAC and the ENGAGE Working Group members for their keen insights and thoughtful deliberations. The findings come from the latest report of the Monitoring the Future Survey, an annual survey of drug use behaviors and attitudes among eighth, 10th, and 12th graders that has been supported by the National Institutes of Health (51视频) for 51 years.</p> <p>Researchers, based at the University of Michigan, Ann Arbor, detected a sharp decline in reported use of most drugs from 2020 to 2021. This substantial falloff was largely attributed to disruptions in drug availability and in the social lives of teens during the pandemic, when many were isolated at home with parents or other caregivers and spending less time with friends. The researchers also found that the percentage of teens currently abstaining from alcohol, tobacco, and nicotine use held steady at historically high levels.</p> <p>鈥淲e are encouraged that adolescent drug use remains relatively low and that so many teens choose not to use drugs at all,鈥� said Nora D. Volkow, M.D., director of 51视频鈥檚 National Institute on Drug Abuse (NIDA). 鈥淚t is critical to continue to monitor these trends closely to understand how we can continue to support teens in making healthy choices and target interventions where and when they are needed.鈥�</p> <p>For the survey, eighth, 10th, and 12th graders self-report their substance-use behaviors over various time periods, including past 30 days, past 12 months, and their lifetime. The survey also documents students鈥� perceptions of harm, disapproval of use, and perceived availability of drugs.</p> <p>The data indicates that, compared to 2024, reported use of most drugs in most grades held steady in 2025. These are some of the key findings:</p> <ul> <li>Abstaining from, or not using, marijuana, alcohol, and nicotine remained stable for all grades, with 91% of eighth graders 82% of 10th graders, and 66% of 12th graders reporting abstaining in the past 30 days.</li> <li>Alcohol use remained stable among all three grade levels, with 11% of eighth graders, 24% of 10th graders, and 41% of 12th graders reporting use in the past 12 months.</li> <li>Cannabis use remained stable among all grades, with 8% of eighth graders, 16% of 10th graders, and 26% of 12th graders reporting use in the past 12 months. Of note, 2% of 8<sup>th</sup> graders, 6% of 10th graders, and 9% of 12th graders reported use of cannabis products made from hemp, which include intoxicating products such as delta-8-tetrahydrocannabinol, in the past 12 months.</li> <li>Nicotine vaping remained stable among all grades, with 9% of eighth graders, 14% of 10th graders, and 20% of 12th graders reporting use in the past 12 months.</li> <li>Nicotine pouch use remained stable among all grades, with 1% of eighth graders, 3% of 10th graders, and 7% of 12th graders reporting use in the past 12 months.</li> <li>Nicotine pouch use remained stable among all grades, with 1% of eighth graders, 3% of 10th graders, and 7% of 12th graders reporting use in the past 12 months.</li> <li>Heroin use among all three grades remains low, though values increased significantly from 2024, with 0.5% of eighth graders (compared to 0.2% in 2024), 0.5% of 10th graders (compared to 0.1% in 2024), and 0.9% of 12th graders (compared to 0.2% in 2024) reporting use in the past 12 months.</li> <li>Cocaine use also remained low and stable for 10th graders, with 0.7% reporting use in the past 12 months; though values increased significantly among the other grades surveyed, with 0.6% of eighth graders (compared to 0.2% in 2024) and 1.4% of 12th graders (compared to 0.9% in 2024) reporting use in the past 12 months.</li> </ul> <p>鈥淭he slight but significant increase we see in heroin and cocaine use warrants close monitoring. However, to put these current levels of use in context, they are leagues below what they were decades ago,鈥� said Richard A. Miech, Ph.D., team lead of the Monitoring the Future survey at the University of Michigan.</p> <p>The results were gathered from a nationally representative sample, and the data were statistically weighted to provide national numbers. The investigators collected 23,726 surveys from students enrolled across 270 public and private schools nationwide from February through June 2025. The treatment, which deploys an expanded umbilical cord blood approach known as omidubicel, was recently approved by the U.S. Food and Drug Administration for use against severe aplastic anemia for patients without options to receive donor hematopoietic stem cell (HSC) transplantation.</p> <p>Nearly all (94%) of patients had rapid neutrophil engraftment, meaning that their white blood cell count had recovered, at median of eight days, dramatically faster than standard umbilical cord transplants. By 100 days post-transplant, most patients had experienced sustained cord engraftment, which means that the transplanted donor stem cells were established in the patient鈥檚 bone marrow and able to produce healthy new blood cells long term.</p> <p>鈥淭he results of this ongoing study are extremely encouraging and indicate a significant advancement in the treatment options for patients with a high unmet medical need. Patients in the study were high-risk but had significantly better than expected outcomes,鈥� said Richard Childs, M.D., assistant U.S. Surgeon General, 51视频 study lead, and scientific director of 51视频鈥檚 National Heart, Lung, and Blood Institute.</p> <p>Severe aplastic anemia causes bone marrow to stop producing adequate blood cells, which leads to infections, bleeding, and ongoing dependence on blood transfusions. If left untreated, the disease can be fatal. Conventional treatment typically involves immunosuppressive therapy and/or HSC transplantation. However, while immunosuppressive therapy can help prolong survival, not all patients respond to it, and many relapse. Lack of a tissue-matched sibling or related donor match remains a major barrier to successful HSC transplantation. In the absence of a donor, patients can undergo umbilical cord blood transplantation, but such transplants often contain low stem-cell numbers, prolonging the time to immune recovery and substantially increasing the risk of infection and rejection of the transplant.</p> <p>Omidubicel stem cell therapy involves taking limited numbers of donated umbilical cord blood stem cells, and culturing them in a lab, where they multiply and are enhanced with a form of vitamin B3, known as nicotinamide, which provides a larger, more effective stem cell dose for restoring the patient鈥檚 blood and immune system.</p> <p>The clinical trial enrolled 18 patients (aged 4-60 years) with severe aplastic anemia who were unresponsive to standard immunosuppressive therapy and without an available donor.</p> <p>Results from two additional study participants have yet to be analyzed. Publication of the complete trial findings are expected next year.</p> <p>The promising trial results prompted the <a href="https://www.fda.gov/news-events/press-announcements/fda-approves-first-cellular-therapy-treat-patients-severe-aplastic-anemia">U.S. Food and Drug Administration</a> to approve the novel transplantation approach for refractory severe aplastic anemia. Overall, the findings, published Dec. 10, 2025, in <em>Nature</em>, describe a previously unknown origin of some aggressive lung cancers.</p> <p>From whole-genome sequencing of more than 1,000 lung cancer cases, the team focused on 542 lung adenocarcinomas with diverse clonal architectures<em>. </em>Among them was a collection of aggressive tumors enriched with the ID2 mutational signature, characterized by a single base pair deletion. Surprisingly, this signature was associated with a type of mobile DNA known as LINE-1 (L1), an ancient part of the human genome that has the potential to wreak havoc by inserting many copies of itself in different places across the genome. L1 is typically silenced within a normal cell but in these tumors was reactivated. L1 reactivation could explain the rapid evolution and aggressiveness of this subgroup.</p> <p>鈥淚t鈥檚 not every day that you discover a new mechanism for aggressive lung tumors,鈥� said lead author Dr. Tongwu Zhang, Ph.D., Earl Stadtman investigator at 51视频鈥檚 National Cancer Institute (NCI). 鈥淭he finding underscores the power of whole-genome sequencing data to reveal genomic structural variants with significance for tumorigenesis.鈥�</p> <p>In addition, the investigators discovered that major driver gene mutations are responsible for varying tumor evolutionary trajectories. Tumors harboring <em>KRAS</em> mutations, which are more frequent in smokers, showed signs of rapid clonal evolution, which helps to explain the aggressive nature of these malignancies. The study underscores that AMD is not one disease and highlights the need for novel treatment approaches.</p> <p>鈥淭his study could help explain why drugs that target just the complement pathway have shown a minimal effect in slowing geographic atrophy,鈥� said Anand Swaroop, Ph.D., chief of the Neurobiology Neurodegeneration and Repair Laboratory at 51视频鈥檚 National Eye Institute (NEI) and a coauthor of the study report. 鈥淚t鈥檚 clear that AMD involves multiple pathways that differentially synergize to generate distinct phenotypes.鈥�</p> <p>Drusen are lipid-rich deposits that develop in the eye's light-sensing retina and are a hallmark feature of AMD, one of the most common causes of irreversible vision loss in older Americans. The development of a few small drusen is normal with age, but AMD risk increases as drusen deposits increase. Typical drusen are located beneath a layer of cells that supports the health and function of the photoreceptors essential for vision, known as the retina鈥檚 pigmented epithelium (RPE). In contrast, reticular pseudodrusen appear above the RPE in clusters that have a distinct ribbon- or dot-like appearance. Presence of pseudodrusen is linked to greater risk of progression to late-stage disease condition called geographic atrophy in which retinal cells die off.</p> <p>The researchers looked at DNA from three groups of people: one with AMD and reticular pseudodrusen (AMD+RPD); one with AMD but no reticular pseudodrusen (AMD-RPD); and a third group with no AMD or reticular pseudodrusen (control). When combined, the two groups with AMD had a greater frequency of AMD-related gene variants, compared to the control group. As with prior studies, there was a disproportionately high prevalence of previously identified AMD risk variants on chromosomes 1 and 10, in both AMD groups, compared to control participants.</p> <p>The picture was different when comparing the two AMD groups to each other. There was no difference between the AMD groups with and without RPD in AMD-associated variations on chromosome 1 Chromosome 1 bears the gene for complement H factor (CFH), a part of the immune system that helps tamp down inflammation. In 2005, scientists first revealed a link between CFH gene variants and AMD and have since vigorously explored how the gene and other components of the immune system鈥檚 complement cascade influence AMD. In 2023, the <a href="https://www.nei.nih.gov/about/news-and-events/news/story-discovery-nei-funded-research-paves-way-new-dry-amd-drugs">FDA approved two new AMD drugs that target the complement cascade</a>. While the drugs slowed geographic atrophy, their effects were modest.</p> <p>In contrast, the frequency of chromosome 10 variations was greater in the AMD+RPD group, compared with the AMD-RPD group, especially with increasing reticular pseudodrusen number. The study found that the reticular pseudodrusen association on chromosome 10 was in a location comprising the genes ARMS2 and HTRA1. In this region, they found an association with variants of HTRA1-AS1 (also known as BX842242.1)鈥攁 stretch of DNA corresponding to a long non-coding RNA. Such molecules affect the function of other genes without first being translated into proteins.</p> <p>鈥淥ur data show that these deposits of reticular pseudodrusen, present in some but not all AMD cases, are driven by pathways associated with chromosome 10 and not by genetic risk in other AMD-related genes such as those on chromosome 1,鈥� said Robyn Guymer, Ph.D., Centre for Eye Research Australia, Melbourne, who together with Melanie Bahlo, Ph.D., The Walter and Eliza Hall Institute of Medical Research, Melbourne, authored a report on the study with an international group of researchers representing the Reticular Pseudodrusen Consortium.</p> <p>The researchers explored how the chromosome 10 risk variants might exert their effects by evaluating eye scans, finding that these variants were associated with thinner retina, compared with controls. While more research is needed, the chromosome 10 variants appear likely to affect retinal structure. </p> <p>"This study reinforces that AMD is not one disease,鈥� said Emily Chew, M.D., who leads the NEI Division of Epidemiology and Clinical Applications and a co-author of the study report. 鈥淥ur findings highlight that different treatment approaches will be needed for different AMD subtypes.鈥�</p> <p>The study was funded in part by NEI. He was a Senior Fellow in the Duke Center for the Study of Aging and Human Development and a member of the Duke Cancer Institute, where he co-led the Neuro-Oncology Research Program. In addition, Dr. Walsh helped to guide research efforts following the 2023 train derailment in East Palestine, Ohio, providing scientific leadership to U.S. Senators in the state and opening the door for important collaborations across multiple federal agencies.</p> <p>Dr. Walsh holds bachelor鈥檚 degrees in molecular genetics and anthropology from The Ohio State University, Columbus. Kennedy, Jr.</p> <p>Dr. Letai takes the helm of the world鈥檚 most prestigious cancer research agency after serving as professor of medicine at Harvard Medical School and medical oncologist at the Dana-Farber Cancer Institute. He possesses decades of experience studying cell death in cancer, developing treatments, and identifying predictive biomarkers.</p> <p>鈥淐ancer, like other chronic diseases, was long neglected in federal research attention,鈥�<strong> said Secretary Kennedy</strong>. 鈥淧resident Trump reversed that neglect, and Dr. Letai鈥檚 leadership of NCI will drive American innovation by focusing squarely on the best science to find causes and cures.鈥�</p> <p>鈥淒r. Advances in understanding cell death and replication are essential to realizing President Trump鈥檚 vision for a healthy America.鈥�</p> <p>Dr. Letai鈥檚 research has been central to bringing venetoclax, a BCL-2 inhibitor, from the laboratory to the clinic. His laboratory work has led to advancements in knowledge of both liquid and solid tumors, as well as a wide range of treatments, including cellular immunotherapies. Dr. Letai is a recipient of the European Cell Death Organization Career Award, the Smith Family Prize for Outstanding Scientific Contributions, and the National Cancer Institute Outstanding Investigator Award.</p> <p>After graduating from Princeton University with a Bachelor of Arts in physics, Dr. Letai received his Doctor of Medicine and Doctor of Philosophy from the University of Chicago. He completed his Ph.D. on the molecular basis of heritable blistering diseases before residency in Internal Medicine at Brigham and Women鈥檚 Hospital and a clinical fellowship in hematology and oncology at Dana-Farber Cancer Institute. Dr. Letai began his studies of programmed cell death in cancer in a post-doctoral fellowship before establishing his laboratory at Dana-Farber Cancer Institute to study how apoptosis can be evaded by cancer cells.</p> <p>Dr. Letai and his wife, Jean, have three children.
